While Google does not specify an exact optimal header size for Google Forms, a common recommendation is about 1600 x 400 pixels at an aspect ratio of 4:1.  

However, you need to keep in mind a few things:  

Centered Focus: Given that Google Forms may adjust or crop the image, ensure that key elements or text are centered so they don't get cropped out.

High Quality: Use a high-resolution image to make sure it appears crisp on all devices, but don't use an overly large file size that might slow down the loading time of your form.

Test It Out: Upload your image to a Google Form to see how it looks and make any adjustments if necessary. Depending on the design and where you want focal points to appear, you may need to iterate on your image's dimensions or content placement.

How to Add a Header Image to your Google Form

To add a header image to your Google Form, simply follow the steps below: 

1. Click on the paint palette icon at the top-right corner.

In the Google Forms editor, look for the paint palette icon in the top-right corner. Click on it.

2. In the theme window, click "Choose image" under "Header."

A sidebar will appear on the right side of the page. This sidebar allows you to modify the theme and background colors. Under the "Header" section, click on "Choose image."

3. Upload or select your header image.

Depending on your preference, you may either: 

  • Select an image from the provided library.
  • Upload an image directly from your computer.
  • Or, import an image from your Google Photos.

Remember, if you're uploading your own image, ensure its primary focus aligns with a 4:1 aspect ratio. This is crucial as you'll have to crop it in the next step. To upload your own image, click on “Upload.”

4. Adjust the image, then click "Done."

After selecting or uploading, adjust your image to fit the header. Once satisfied, click "Done" to set it as your form's header.
