The legal business name is the formal registered name of the company, while the “doing business as” (DBA) name is any trade name the company uses publicly.
Section C collects information about the foreign worker, including their name, address, country of citizenship and immigration status.
Section D outlines the job opportunity details, such as job title, duties, minimum requirements and worksite location.
