U.S. citizens and nationals applying for their first passport, minors and individuals whose previous passport was issued more than 15 years ago must use Form DS‑11.
Complete the form online or by hand, gather required documents, photos and fees, and submit them in person at an authorized acceptance facility.
Acceptable evidence includes a U.S. birth certificate, naturalization certificate, consular report of birth abroad or a previous fully valid U.S. passport.
Provide a government‑issued photo ID such as a driver’s license, along with a photocopy of both sides of that ID.
Photos must be in color, measure 2×2 inches, have a white background and be taken within the last six months; glasses are not permitted.
